Treatment agreement

In this treatment agreement, there are a number of agreements regarding the proceedings in our practice Andriette & Henk. The purpose of this treatment agreement is to ensure clarity on the expectations of the therapy process from both sides.

Therapists

  • The therapists at Andriette & Henk are Andriëtte Nommensen and Henk van Zeijts.

Quality Care

  • Andriëtte and Henk are both affiliated with the following professional associations: the Dutch Federation of Healthcare (NFG) and the Register of Complementary Healthcare Practitioners (RBCZ). Privacy

  • We make every effort to safeguard your privacy, handling personal data with care. In all cases, we adhere to applicable laws and regulations, including the General Data Protection Regulation (GDPR). You can read the privacy statement on our website.
  • Our online client environment is accessible through our website. This environment complies with legal requirements and can be used for sending (secure) messages, filling out questionnaires, and sharing processing assignments. You will receive a request from us to complete your details for this system.

Documentation and Information Exchange

  • In the intake and on the intake form, you provide all relevant information. Any adverse consequences resulting from withholding information, whether or not present in the files of doctors and/or other practitioners, are your own responsibility.
  • Any consultation with third parties for the purpose of guidance only happens after consultation with and consent from you.
  • If you, a practitioner, or a general practitioner request a written report of the guidance process, you will receive an invoice for preparing this report. This invoice is for a maximum of one hour of work at the rate agreed upon in this agreement per session.

Guidance

  • Together with you, we determine the treatment method and the goal of the guidance process. You describe your request for help in the online intake form, for which you receive a digital invitation.
  • Interim evaluations of the guidance take place. This is decided jointly, usually the first one after about six sessions.
  • You are and remain solely responsible for your own health.
  • Andriëtte and Henk provide relationship guidance together.

Rates, Payment, and Reimbursement

  • The rates are listed on our website. These can be reassessed annually. Rates can be adjusted per calendar year. If this is the case, you will be informed at least 2 months before the rate change takes effect for you. For ongoing processes (counted from the intake), the annual rate increase takes effect on July 1st of that calendar year.
  • You will receive a digital invoice after each session and must pay it within 21 days of the invoice date.
  • Most health insurers reimburse (part of) the costs of individual guidance in the supplementary package. These costs usually do not fall under the deductible of your health insurance. An overview of reimbursement for psychosocial therapy by health insurance can be found on the website of our professional association, the NFG. In case of doubt, it is best to contact your health insurance company.

In Case of Hindrance

  • If you are unable to attend, please inform us as soon as possible so that we have the opportunity to schedule another client. If the appointment is canceled less than 24 hours in advance, we will charge for the reserved time.

Termination of Therapy

  • The therapy can be terminated at any time by mutual agreement or unilaterally by the client or therapist. To conclude the therapy process, a reflection session takes place in a concluding session.

Agreement and Approvement

  • The purpose of this treatment agreement is to clarify expectations for both parties. By approving this document, you declare that you have read the treatment agreement and agree with it.
